I hereby authorize METASTAR AIRLINES to apply for the VIP service provided by Huan Yu VIP Terminal on my behalf. I have understood the following precautions and have confirmed that the information provided on this page is correct. 

Guest Reservation VIP Terminal Service Notes:
Please complete the form reservation no later than 4 hours before your flight departure (service hours are from 8:00 to 20:00 daily). You can also contact the dedicated hotline at 02-77098838 #8000, and our customer service team will confirm the reservation details with you and request relevant information.

Please note the following precautions:
1.All service facilities provided in Huan Yu VIP Terminal are reservation-based with a usage time of three hours. Due to limited capacity, Huan Yu VIP Terminal will arrange the usage of private rooms or lounge according to the situation on-site. If you need tax refund services, please inform METASTAR AIRLINES customer service team and arrive at the Huan Yu VIP Terminal one and a half hours before your flight. For more information on other airlines' check-in notices, please refer to the Taoyuan International Airport official website at http//www.taoyuan-airport.com and click on "Flight Information."

2.Huan Yu VIP Terminal is located between Terminal 1 and Terminal 2, within airport control area. Drivers and accompanying guests must apply for a temporary permit. Thus, please bring relevant ID or documents and make sure no dangerous goods are carried into the control area.
(Note 1: Passengers should bring passport and relevant travel documentary with them; others should have ID and Driver license at hand.)
  
3.If there are changes to passengers or accompanying persons or vehicle information, please inform MetaStar Airlines at least 4 hours before departure/arrival time for smoother service for the passengers.
 
4.If passengers could not travel due to travel documents or cancel reservations four hours within departure/arrival time, the VIP Terminal service will be deemed as used.

5.TIAS and EGAS are entrusted with baggage handling by Huan Yu VIP Terminal. Passengers are advised to check the appearance and contents of their luggage before check-in. If any abnormalities are found, please immediately inform the on-site staff for help with compensation from the relevant parties. To ensure that inbound passengers receive their own baggage, it is recommended to check in the baggage in their own names. After making a reservation, Huan Yu VIP Terminal is allowed to inquire about the passengers' checked baggage information from the airline company.

6.Tax refund: Passengers shall bring the items, forms, receipts with them on the date of departure to claim tax returns at Huan Yu VIP Terminal.

8.No refunds will be given after the sale of VIP Terminal service package through METASTAR AIRLINES. The service package is valid for one year from the date of purchase. If the service package is not fully used by the date of expiration, the rest benefits will be forfeited.

Personal information collected through this sheet will be used solely for services of Huan Yu VIP Terminal.  According to Article 3 of the Personal Data Protection Act, applicants can request to delete, stop processing and using of personal data, and MetaStar Airlines will not be able to provide reservation service. 

If the Agreed button is checked, it suggests that:

1. applicants agree to authorize MetaStar Airlines to apply for Huan Yu VIP Terminal services;
2. personal data will be used solely for application for services of Huan Yu VIP Terminal;
3. if the above personal information is mis-typed by applicants and therefore results in invalid reservation, compensation for losses is not applicable;  
4. applicants agree with the plan selected above and related regulations about reservation and shuttle services.
